If you have any problems printing the forms, just call our office and our staff will be happy to email or fax them to you.  

In addition, we welcome any additional information about your medical history, including lab results, doctors’ commentaries, medical records and/or list of current medications and dosages, which will aid in the evaluation of your medical history and previous pain therapy efforts.

48-Hour Calmare Appointment Cancellation Notice:

Calmare Therapy performed exclusively by certified provider Dr. Michael Cooney is in high demand and his appointment schedule is often fully booked.

Because we are carefully scheduling scrambler therapy treatments every day, it is imperative that patients let us know if they cannot:
• Begin the 10-treatment regime on their scheduled day and time
• Keep a pre-scheduled appointment for any of their 10 treatments

By not doing so, we are forcing another patient in pain to wait even longer for treatment, which we never want to do.

With this said, patients who confirm any appointments with Calmare Therapy NJ must cancel them at least 48 hours before their scheduled treatment time.

Failure to do so will result in a service fee of $250 per missed session.
